Overview

This report is to review all YHDP specific questions and their responses during a selected report period.

A Looker license is required to run this report. This report printed in PDF format is used to submit this report.

4. WI BoS YHDP Assessment Report

To Run the Report:

Enter the reporting period date for the quarter.
1. Click the box for the Reporting Period Filter and select "in range"
  1. Choose the Start Date and End Date.
    1. End date should be plus one date, Example: 7/1/22 - 10/1/22 would be the first quarter report dates
Enter the Name(s) of your agency's programs to be included in the report

All other prompts should be left as is.

To Print the Report:

Click the 3 dots and select "Download"
