[RFRL-110] VI-SPDAT Referral Detail

Report Purpose and Summary

The VI-SPDAT Referral Detail report returns a list of clients assessed during the reporting period with the VI-SPDAT score details. All users can run the report. There are restrictions for which information can be accessed based on the rights of the user.

Instructions

Make sure you are operating in Clarity under the agency you are running the report for. You can check and change this under your name in the top right corner.

Step 1: Click on the Launchpad and select the Reports icon.

Step 2: Once on the Reports main page, click the down arrow to expand the Community and Referrals section. 


Step 3: Click the Run button next to [RFRL-110] VI-SPDAT Referral Detail Report.

Reporting Parameters

Step 4: Enter the following parameters:

Referral Type

Choose All, Community Queue, or Agency Referrals

       All both community queue and agency referrals

       Community Queue the client first being referred to a community queue

       Agency Referrals Direct referrals (the client is referred directly to the program)

Report Date Range

Select your start and end date for the report range. Referrals that were created within this date range will appear in the data table.

Report Output Format

Choose Web Page, PDF, or Excel

Step 5: Click submit


The file will be sent to the Report Queue. Once the report is processed, you will receive a notification that the report is ready.


To open the report, either select Open on the popup or click the Queue icon and then click Open.


Reporting Details

The information in this report is pulled from service placements. The report provides the following information:

  • Client Full Name
  • UI
  • DOB
  • Agency
  • VI-SPDAT
  • Days Pending
  • Contact Info inputted (yes/no)
  • Location Info Inputted (yes/no)
  • Last Check-In
  • Notes inputted (yes/no)
