Madison / Dane CoC YHDP Projects
Overview
Youth Homelessness Demonstration Program (YHDP) funding supports three different project types in Madison / Dane County CoC: System Navigation, Transitional Housing (TH) and Rapid Rehousing (RRH). Funding for the TH and RRH projects are combined into what HUD calls a Joint TH / RRH project that comes comes with some unique data entry requirements in HMIS.
- System Navigators will follow the Supportive Services Only (SSO) workflow in HMIS.
- YHDP funded housing projects follow the Joint TH / RRH workflow.
In most situations a client should not be enrolled in a System Navigation project and a YHDP housing project at the same time. Standard workflow is for folks to initially connect with the system navigation project and then, when appropriate, be referred to the housing project for more robust support. Once they are referred and connected with housing they should be exited from the System Navigation project.
YHDP Referrals
There are two situations where HMIS users may use the Clarity Direct Referral process to either help connect a young adult with a YHDP provider or to document in HMIS that a connection has been made.
- Street outreach and emergency shelter providers working with young adults can connect clients with the YHDP funded system navigation projects to access additional supports and services.
- Some YHDP funded system navigation projects use HMIS direct referrals to document and track referrals made to the YHDP funded joint TH / RRH projects.
Documenting YHDP Services
In addition to the collecting information about a participant when they enter and exit the project, the Dane County YAB has developed a set of services for projects to track in HMIS. These services reflect the type of assistance staff are providing to clients and align with the core goals of the Coordinated Community Plan and may be used to assess program success and support system development and evaluation. It is therefore expected that staff enter a service following every meaningful interaction with a client a client.
