Madison / Dane County CoC Coordinated Entry - Housing Services

The follow trainings and resources are available to support users of the Madison / Dane County Coordinated Entry system:

Tier-1: Initial assessment and Shelter and Street Outreach enrollment

Overnight shelters and street outreach should collect responses to the Madison / Dane CoC CE enrollment fields on their project enrollment screen in Clarity HMIS. This information, along with a households HMIS history, is used for Tier-1 prioritization and if these fields are not complete, accurate, and current, the household will not be appropriately considered for CE housing opportunities. Monitoring and reporting

Tier-1 Data Completeness - Use this report to assess data completeness for the Madison / Dane CoC Coordinated Entry question on Street Outreach and Emergency Shelter enrollment screens that are used for Tier-1 assessment and prioritization.

Unfulfilled Tier-2 Invitations -  Use this report to track clients with a Referral to scheduled Coordinated Entry Housing Needs Assessment. It reports the number of completed and unfulfilled Tier-2 invitations along with client details to help ensure referred clients in Street Outreach and Emergency Shelter are not missing housing opportunities.
