Dane CE - Unfullfilled Tier-2 Invites

Background

All active clients in Street Outreach and overnight Emergency Shelters are included in Tier-1 and assessed based on their HMIS history and responses to questions on their entry screen. Agencies working with a client prioritized for tier-2 are emailed invitation and are asked to connect with the client, complete the tier-2 assessment, and refer them to the appropriate community housing queue. Agencies should attempt to contact the client in each of the first three weeks following the invitation and document these efforts on the referral in the client's HMIS history. If an agency is unable to contact the client they should coordinate with other relevant providers and as necessary notify the CE manager in accordance with the procedures in the Dane CE workflow. The Dane CE - Unfulfilled Dane Tier-2 CE invitations report is designed to allow agencies, funders, and the CE manager to monitor the progress of these invitations and help ensure clients are not "slipping through the cracks."

Running the report

A Looker Data Analysis reporting license is required to access this report in Clarity. After locating the reports page, navigate to the Data Analysis tab and scroll down to the Dane CoC section. There you will find the Dane CE - Unfulfilled Dane Tier-2 CE invitations report.
