Madison / Dane Re-imagine CE questions - Workflow and Recorded Training

Overview

Seeking a more efficient, equitable and trauma informed approach, the HSC is updating the prioritization process for adults seeking housing assistance through the CoC's Coordinated Entry system for adults. For additional information on this work please refer to Reimagine CE Workgroup's page on the HSC website.

The biggest change will be implementing a two-tiered approach with all individuals and families enrolled in a Street Outreach or overnight Emergency Shelter being screened for the initial tier. To facilitate this, the HSC is adding a few additional questions to the enrollment screens at these projects for staff to ask clients as they are enrolling. Responses will help help determine who is prioritized for the communities limited CE housing options and, for most clients, avoid a more intensive questionnaire such as the VI-SPDAT.

Important - As with other questions on the enrollment screen and VI-SPDAT, these new questions cover sensitive topics and it is essential that staff solicit responses with a respectful and trauma informed approach. As always, clients may choose not to answer and it will not impact their ability to receive services from the shelter or outreach project. That said, if a client chooses not to answer it will impact their position on the prioritization list and may prevent them from accessing housing resources that would otherwise be available to them. This makes it important that staff do their best to work with clients to collect the most complete and accurate responses possible. 

Changing and updating responses over time

There are two main situations where staff should update responses to these fields:

  • A client initially declined to answer a question but is now willing to share
  • A client's situation has changed since initially answering the questions

These questions only exist on the enrollment screen, so any and all changes must be made there. This is different than the guidance for most enrollment questions, for which updates are documented on status update screens.

Important - If a client is actively enrolled in more than one street outreach or emergency shelter project the prioritization report will only pull from the most recent enrollment. This makes it necessary that the most accurate and complete information is entered on the most recent enrollment and that agencies coordinate to ensure this happens. When updating an existing enrollment, the best practice is to update all active enrollments.

With a few exceptions, these fields will appear on the enrollment screen for the head of household of all clients enrolling in Street Outreach or Emergency Shelter in the Madison / Dane County CoC. If you have questions about why these fields are, or are not, appearing on a screen, please contact your system administrator.

Family size and barriers to housing questions

The following set of questions appear after the Prior Living Situation questions.

Important: The first two questions about the number of adults and children will be used to determine whether a household will be considered for the singles list of the families list in for CE referrals. If a clients reports that there will be zero children (ie youths age 17 or younger) who would be living with them they will be considered for the singles list.

DV shelter use questions

The following questions use conditional logic and only appear if a client responds yes to to the question about Survivor of Domestic Violence. Additional logic determines which of these questions appear.

Important: In HMIS a person is considered to be a "survivor" even if they are still involved in a DV relationship or situation.
