YHDP BoS System Navigator and Housing Navigator

Overview

The Balance of State has two Service Only projects related to YHDP. The BoS System Navigator assists youth in connecting with services. The Navigator is associated with housing outcomes and assisting youth in obtaining housing.

Program Enrollment

Enrollment should reflect the actual date the client begins working with the program.

Youth need to meet homeless criteria for categories 1, 2 or 4.
Enrollment date should match the date the homeless verification is completed.

Assessment for YHDP BoS System Navigator

Complete the YHDP BoS System Navigator Assessment at the time of program enrollment.

To make updates, edit the existing assessment and click "Save."
