New Agencies - Requesting Access to HMIS

Agency Participation Requirements:

Read and adhere to the following documents in full:

HMIS Policies and Procedures Manual
Agency Partnership Agreement
HMIS Release of Information
Consumer Privacy Notice (must be posted where HMIS data is collected)
Documentation of verbal and/or written client permission

Program Set Up:

After all requirements are met and the agency is approved, the agency may request access to the system by contacting ICA.

Cost:

Agencies are charged a user license set up fee of $300 for each staff member accessing the system. For programs whose funding subsidizes HMIS funding, the annual cost of each user license will provided at a lower rate. Please see the HMIS Policy and Procedure manual for the subsidized rate. User license invoices are for April through May each year and are sent out each year in mid to late summer.
