DHS 1915i Program

The DHS 1915i Program for Housing and Support Services is administered by the WI Department of Human Services, and addresses the need for homelessness assistance and supportive services. For more information about program requirements, please visit DHS's website.

Agency's wishing to participate must have a current HMIS Agency Participation contract signed and be approved by their HUD CoC.

DHS 1915i Specific Data Elements

  • Only adults who qualify for 1915i need to be enrolled in the 1915i program.
  • In addition to the HMIS Core Data Elements, 1915i programs are required to collect the following data elements for all eligible adults.

Is this adult 1915i eligible? Recorded at program entry.
Total household monthly income:
Regardless of adults eligibility, when there is more than one adult in the household you must TOTAL BOTH/ALL adults income.
Recorded at Program entry.
Updated as needed at program status update and/or program exit.
Are you currently receiving mental health treatment services? Recorded at program entry.
If yes: What types of of mental health treatment services are you receiving? Recorded at program entry. For a list of mental health services tracked in HMIS, click HERE.
Are you currently receiving substance use treatment services? Recorded at program entry.
If yes: What types of substance use treatment services? Recorded at program entry. For a list of substance use treatment services tracked in HMIS, click HERE.
Date Housed Recorded at program status update.* Dates must match*
Monthly Rent Amount Recorded at program status update once client is housed.
Zip Code of Rental Unit Recorded at program status update once client is housed.

  • Enrollment Date: The client’s enrollment start date in HMIS needs to match the date the agency submitted the prior authorization request
  • *Date Housed must match at least one Status Update.
  • Annual assessments are also required at the anniversary mark of client being enrolled in project. Date Housed may be recorded on the annual assessment date if that is also the date client becomes housed. 
  • Exit Date: The client's exit date would be at the end of the client's membership year if not reauthorized.
    • If contact is lost with client, the enrollment should be end dated within 60-90 days.
    • Client's exit date can be removed if they re-engage with the program within their membership year as they would still be eligible for a full year.
