Milwaukee HMIS Coordinated Entry Workflow Training

The Coordinated Entry process is a structured system overseen by the Milwaukee Coalition on Housing and Homelessness (MKE CoC). MKE CoC mandates that staff undergo specific training on Coordinated Entry and how to conduct assessments. ICA offers training exclusively to agencies and users who already have access to HMIS and will be responsible for adding households to and removing households from the HMIS prioritization list. HMIS plays a crucial role in the Coordinated Entry system, serving as one of the tools used to prioritize clients.

  • Access to HMIS is essential to proceed with the CE portion. 
  • To refer individuals to the Housing Prioritization List, it is crucial to have completed the appropriate trainings. 
  • You have the flexibility to watch the video as many times as necessary, so take your time to grasp the content.

  • To ensure credit for completing the training, be sure to fill out the Training Completion Form at the end of the video.

Required Steps for Referrals

For your Referral to be finalized, you must complete the following steps:

  • MKE CEVA (Coordinated Entry Vulnerability Assessment).
  • Homeless History Tracking Form (upload to Files).
  • Upload Third Party Homeless Verification letters (MUST be on letterhead).
  • MKE Coordinated Entry Update and Notes – All UPDATES must be tracked in Clarity.
  • Status Update Assessment – Changes in Living Situation.
  • Current Living Situation – Street Outreach only (EVERY CONTACT).
  • Add a Note (On the general tab).
  • Client MUST be updated every 30-60 days. 

Kindly ensure that program applications are submitted within two weeks of client assignment. Failure to provide the necessary documentation promptly will lead to the consideration of the next client for housing placement.
