Death and Deceased Documentation

Death and Deceased Documentation is now available in HMIS on the Client Profile Tab.

If an agency is aware a client is deceased, the user will toggle on the indicator and document the client's Date of Death and how the agency was made aware of the death.

  • Date of Death should be as close to known date as possible
  • If a client dies while enrolled in a program, the destination of the program enrollment should stated "Deceased" and the client profile indicator should be toggled on.
