Creating a Program Enrollment 

  1.  Before creating a program enrollment you will first need to search for the clients Clarity profile. Depending on their history you may need to create a new client and/or manage the household.
  2. The Programs tab contains information about current and past Enrollments. 
  3. The top section of this tab includes the Program History. This is a list of the programs the client is either currently enrolled in or was enrolled in previously.
  4. The bottom section of this tab is the Programs Available list. This includes the active HMIS programs available at your Agency for client Enrollment. 
  5. To Enroll a client/household into a program select the small down arrow to the right of the desired Program. 
  6. Next, if applicable, toggle on the other Household members to include in a group enrollment. 
  7. In the example below we are enrolling Petenwell, Elkhart, and Boulder into the Generic PSH program. 
  8. Click Enroll.
  9. After clicking enroll you will be prompted to fill the client enrollment screen which collects information about the client including their disability status, Income,  etc are answered specific enrollment questions for each household member.
  10. Pay particular attention to the Project Start date and ensure that you are using the date the client enrolled and not the date the you are doing the data entry. 
  11. It is also important to carefully explain and collect accurate Prior Living Situation information.
  12. Permanent Housing projects will see a field for the Housing Move In Date (HMID). If this does not coincide with the Program Entry date this should be left blank and filled in once the HMID becomes known.
  13. Emergency Shelter and Street Outreach projects will see a date of engagement field. This field may be left blank until the client is fully engaged.
  14. Other funding sources and project types, including those funded by the VA, RHY, SAMHSA, or DEHCR HCMS may have additional enrollment screen questions.
  15. Once all of the fields are complete, you will be prompted to save, or move on to the next household member. If there are any missing fields, Clarity will require you to return and provide a response.
