Annual and Status Update Assessments

These two Assessments provide a place for staff to update information that may have changed since the initial enrollment into a project. Examples include change of income, benefits, or insurance, updated Chronic Homelessness status, etc. Updates are required at least once annually and anytime a clients information changes during a program enrollment.

Annual Assessment - required on or within 30 days post/prior to the anniversary date of the Head of Households enrollment start date.

Status Update Assessment - required any time the client's information related to income, non-cash benefits, or health insurance changes. It is also may be required in some Coordinated Entry projects to update changes to the client's residence, number of times and number of months homeless

Status Assessments are one of the options listed under a Program Enrollment’s Assessments sub-tab. This can be accessed in two ways: 

    1. Clicking the Assessments Sub-tab under the Program tab.
    2. Clicking on the plus sign next to the Status Assessments list on the right-hand side of the client’s file.

Note: the top-level Assessments tab allows viewing of all the Assessments associated with a client’s file whereas the sub-tab Assessments is the preferred place to complete a new Assessments related to a specific Program. It is not possible to compete a Status Update or Annual Assessment on the main Assessments tab.

 2. Click Start next t the desire assessment,

 3. Make any desired updates to the fields. In most projects previous data will cascade through so you only need to make update field that you know have changed.

 4. Save the assessment.

Notes:

  1. An Assessment cannot be marked as Private while creating it. This must instead be done after the fact using the Edit function.

  2. As this time an Assessment cannot be set to Private from within the Program Enrollment. Instead Edit the Assessment by navigating to the Assessment record under the Client’s primary Assessments Tab.

  3. Annual and Status Assessments cannot be set to Private in this way. They are tied to the Enrollment and will only be Private if the Enrollment itself is set as Private.
